Connected by Hope is a digital platform designed to help caregivers access guidance, support, and connection through shared lived experience. Caregivers navigating complex situations often feel isolated and unsure where to turn. Connected by Hope brings together real-time video conversations, educational resources, and guided entry points into a single, supportive experience. I led the end-to-end design of the platform, shaping the user experience across enrollment, navigation, and live connection flows.
The Challenge
Caregivers often struggle to find relevant, trustworthy support at the moments they need it most. Existing resources are fragmented, difficult to navigate, or lack the human connection needed to feel truly helpful. The challenge was to design a platform that could make support feel accessible, personal, and easy to engage with, while guiding users toward meaningful connection without overwhelming them.
The Solution
I designed Connected by Hope as a structured, multi-touch platform that connects users to support through guided pathways and real-time interaction. The experience balances human connection with digital structure, allowing users to explore resources, connect with care partners, and take action at their own pace. Key design decisions included: • Guided entry over open exploration: Designed clear pathways to help users quickly understand where to start and what actions to take • Human connection as a core feature: Prioritized video-based interactions to create more meaningful and trustworthy support experiences • Flexible engagement model: Allowed users to move between self-guided resources and live interaction based on their needs • Integrated enrollment flow: Designed onboarding to reduce friction while connecting users to the broader platform experience • Clear action hierarchy: Structured navigation and CTAs to guide users toward key actions without overwhelm
The Process
My process focused on designing a cohesive platform experience that connects multiple touchpoints into a clear and intuitive journey: • Defined user needs and key caregiver scenarios • Mapped platform structure and user flows • Designed enrollment, navigation, and connection experiences • Created high-fidelity UI and supporting assets • Partnered with stakeholders to align on requirements and implementation The result was a platform that made it easier for caregivers to access support, reduced friction in connecting with care partners, and created a more structured, approachable experience for navigating complex situations.
